Return to this form as many times as required to enter data on however many families you want to document.
The blue fields are required.
Enter as much data as you know. Please use these formatting guides:
- Enter locations as: City, County, ST, Country
  (e.g., Crawford, McLennan Co., TX, USA)
- Enter dates as dd Mmm yyyy
  (e.g., enter July 6, 1946 as 06 Jul 1946)
Do not enter an unmarried person as the husband or the wife. Instead, submit a form that includes that person as a child of his/her parents.
If there are more than 10 children, please submit a second form for the 11th and subsequent children.
(If you have to do this, enter the parents’ names on the second form, enter the childrens’ data, and add a comment in the 'Notes' field which states that child "#1" is really child #11.)
Please list children in birth order.
